We suggest that you go to the following page and fill out the first four sections only of the medical exam. The link is below:

When you have finished (and when you have made an appointment with us), please send the form to:

immigration@wallstreetphysicians.com

When you arrive for your appointment, please bring the following materials with you:

  • All medical records, including vaccination records, translated into English if necessary
  • Previous Diagnostic tests such as chest x-rays
  • Your personal identification with photo
  • Your Passport and VISA
  • Verification of vaccination status

The immigration medical examination will be completed in one visit unless vaccines and/or chest x-ray are needed.

Payment for the immigration exam is due at the time of your appointment; we accept cash and major credit. The cost of the exam is $350.00. That does not include the cost of the blood work or the vaccines or chest x-ray. Also, it does not cover the cost of any necessary treatment if Dr. Carron finds that you have a sexually transmitted infection. If you have medical insurance, it may cover the lab work. One way or another, you will get a separate bill from the lab.

All required vaccinations are always stocked in the Wall Street Physicians offices so you can receive the vaccines you need as soon as possible. If you need a chest x-ray, you can have it done in the same building. That will be billed separately by the radiology facility.

In most cases, you can anticipate receiving your completed paperwork 7-10 days after your first appointment.

We will supply your completed I-693 in a sealed envelope to deliver to USCIS. A copy of your completed I-693 will be supplied for your personal records as well.
